DCSD Community,
Thank you for your support of the COVID-19 testing effort. While we are not currently required to perform random sampling, we are very happy to be prepared for the eventuality of surveillance testing.  This consent will also allow School Nurses to test students who exhibit symptoms consistent with COVID-19 at any time. If you have not turned in a consent form for your student, an electronic version is now available:
